Why does Satoko keep killing Rika?

The reason why this happens, is because Satoko steals Sample 173 from Takano and uses it on whoever she wishes in order to kill Rika. She uses it on Oishi at the end of Watadamashi Hen and during Nekodamashi she uses it on Mion’s mom, the mayor and on Akasaka as well.

Rika Furude
Rika Furude is the true main protagonist from Higurashi: When They Cry and the deuteragonist of its sequels Gou and Sotsu. She is the daughter of the shinto priest in Hinamizawa. She is the miko, or shrine maiden, of the Furude Shrine, appearing in miko garb for her part of the Watanagashi Festival.

Why does Rika leave Hinamizawa?

Refusing to listen, Satoko strikes Rika and mentions her secret desire to leave the village as a reason she deserved the curse put on her. This only furthered her desire to leave Hinamizawa once she had changed her fate in the “Festival Accompanying” arc.

The “Unknown Medicine” in this series is the medicine that several characters try to inject into one another. In reality, this is C-120 (an alternate version of it is named C-103), a treatment for Hinamizawa Syndrome. It usually appears as a syringe full of golden, orange or colorless liquid.
