The tracker bug represents Smith making a change to Neo’s coding that allows his location to be traced. Because Smith and his men are able to bend the Matrix’s rules, the tracker can be pushed painfully through Neo’s stomach, but this isn’t strictly necessary.

Is Neo a Programme?

Neo is not a program. The One was a genetically altered human being who had his/her sensory pathways altered by the machines. In a sense, (s)he was a genetic experiment who, when the Matrix code signals were fed into his/her brain, could hack into that signal, even modify it at will.

In Neo’s case the Agents were attempting to locate Morpheus, so they needed to know Neo’s location when he met with Morpheus. Since the Agents can’t distinguish Neo from all the other bluepills and Morpheus could meet Neo anywherein the Matrix, they need to mark Neo with the bug.

If Morpheus picks up Neo to unplug him but blindfolds Neo (and no other bluepill sees Morpheus or Neo) then the Agents can’t track Neo’s location. The tracking bug removes the limitation that the Agents require bluepills to seesomething.

At this point in the film, Neo is still connected to the Matrix, so the Agents still have control over his body and can make modifications. Agents are computer programs and Neo is still linked into the program. It’s possible that some other program which is confederate with the Agents is what seals Neo’s mouth.
