Either the bullet tore through the soft palate of his mouth and went through his brain, causing Hulk to come out and then the damage was healed. The bullet began to leave the barrel, but before it did, Hulk came out and the bullet just ended up hitting the back of his throat/top of his mouth and he spit the bullet out.
