How is a positive Coombs test treated?

Treatment calls for high doses of steroids such as prednisone, which may be supplemented with immunosuppressive medications such as azathioprine (Imuran). There are two types of Coombs’ tests: direct and indirect.

What does a positive Coombs test indicate?

An abnormal (positive) direct Coombs test means you have antibodies that act against your red blood cells. This may be due to: Autoimmune hemolytic anemia. Chronic lymphocytic leukemia or similar disorder.

Does Coombs positive go away?

Most babies that are Coombs (DAT) positive go home at the usual time. It is possible that the anemia and jaundice may worsen after your baby has gone home. So, your baby will need to be seen again within a few days of going home.

What is RhoGAM and how does it work?

RhoGAM is a medicine that stops your blood from making antibodies that attack Rh-positive blood cells. RhoGAM is a sterilized solution made from human blood that contains a very small amount of Rh-positive proteins. These proteins keep your immune system from making permanent antibodies to Rh-positive blood.

How does a Coombs test work?

It detects antibodies that are already attached to red blood cells. The indirect Coombs test is done on a sample of the liquid part of the blood (serum). It detects antibodies that are present in the bloodstream and could bind to certain red blood cells, leading to problems if blood mixing occurs.

Is Coombs test positive in ABO incompatibility?

Although the indirect Coombs test result (neonate’s serum with adult A or B RBCs) is more commonly positive in neonates with ABO incompatibility, it also has poor predictive value for hemolysis.

Rhesus disease can largely be prevented by having an injection of a medication called anti-D immunoglobulin. This can help to avoid a process known as sensitisation, which is when a woman with RhD negative blood is exposed to RhD positive blood and develops an immune response to it.

How do you administer RhoGAM?

Administer at a frequency of 600 mcg (3000 international units) IV every 8 hours or 1200 mcg (6000 international units) IM every 12 hours until the total dose is administered. The total calculated dose should be administered within 72 hours of the exposure.
